A 29-year-old man was ejected from a car in a high-speed motor-vehicle crash without loss of consciousness. Upon arrival at Harborview Medical Center (HMC) he complained of neck and lower abdominal pain. Per HMC trauma protocol, he underwent a standard radiograph trauma series (anteroposterior chest radiograph, lateral view of the cervical spine, and anteroposterior view of the pelvis), which showed multiple fractures of the cervical spine. Physical examination demonstrated gross hematuria. The patient was stable and was sent for computed tomography (CT) angiography of the chest and neck followed by a venous phase scan of the abdomen and pelvis. Cervical CT confirmed multiple unstable factures of the cervical spine, cord compression, and a left vertebral artery stretch injury. A small amou...

How patients make decisions about their future treatment has been sparsely study and with respect to limb amputation, a particularly difficult decision, not at all. An examination of this should furnish nurses vital knowledge about how patients come to the decision to give or refuse this consent. To reach as deep understanding as possible of how from the patients’ point of view they reach the decision to consent to the amputation of a lower limb. The research was conducted in the qualitative method. Thirty lower-limb amputees (aged 32-88) took part in the study. In-depth interviews were held with the participants. The data were processed by means of content analysis. The main thematic categories identified were, in the chronological order of their appearance: ‘The trail of torment leading...<div id="medworm"><p><b><i>MedWorm Sponsor Message:</i></b> Have a look at <a href="http://www.theorthopedicsdaily.com">The Orthopedics Daily</a>, the new orthopedics portal powered by MedWorm, with all the latest orthopedics news and research updated daily.</p></div>

Discharge Criteria: Does a Revised Discharge Scoring Tool Accurately Reflect Patient Readiness for Discharge From Phase I PACU?

Post Anesthesia Care Unit (PACU) nurses often assess patient readiness for discharge using the Post Anesthesia Recovery Score (PARS), or “Aldrete” tool. However, PARS does not include data on pain, nausea, heart rate, and temperature that could slow discharge. The purposes of this study were to compare the effectiveness of a revised PACU discharge scoring tool that includes pain, nausea, heart rate, and temperature with the current PARS tool in time to readiness for discharge from a Phase I PACU and to assess reliability of the new tool compared to PARS. Both the PARS and new tool criteria are measured via objective assessment on a 0-2 point scale and use a sum score of all criteria to determine discharge readiness. 1 variable on the new tool differs for general and spinal anesthesia: ...

Humor—a comic, absurd, or incongruous quality causing amusement. In the midst of a very serious time in our country and the many non-humorous events that are occurring, I have decided to consider how humor is a vital part of caring for oneself. Certainly there are physical benefits to humor: a stronger immune system, reduced blood pressure and heart rate, reduced stress hormones, reduced pain, and muscle relaxation. Think about how relaxed you feel after a long “belly-laugh.” I don't know about you, but I can hardly move. Mentally, humor can decrease stress, reduce anger and anxiety, increase joy, establish a more optimistic outlook, foster a sense of control, and change our perspective.
SummaryNeurons located in the trigeminal subnucleus caudalis (Vc) play crucial roles in pain and sensorimotor functions in the orofacial region. Because of many anatomical and functional similarities with the spinal dorsal horn (SDH), Vc has been termed the medullary dorsal horn ‐ analogous to the SDH. Here we report that when compared with embryonic SDH neurons in culture, neurons isolated from the Vc region showed significantly slower growth, lower glutamate receptor activity and more cells undergoing cell death. SDH neuron development was inhibited in co‐cultures of SDH and Vc tissues while Vc neuron development was promoted by co‐culture with SDH tissues. Furthermore, we identified that small (non‐protein) ninhydrin‐reacting molecules purified from either embryonic or post‐...

Mustard seeds have been used in traditional folk medicine as a stimulant, diuretic, and purgative and to treat a variety of ailments including peritonitis and neuralgia. Mustards are still used today in mustard plasters to treat rheumatism, arthritis, chest congestion, aching back, and sore muscles. To make a mustard plaster, mix equal parts of flour and powdered mustard and spread it as a paste on a doubled piece of soft cloth. Apply mustard plaster to the affected area for a maximum of 15 minutes. Prolonged application can result in burns to the skin and nerve damage. Skin lesions occur within hours after exposure, and there is no significant therapy procedure. This case report is about a patient with second-degree burn, occurred when a mixture including mustard seed was exposed to her s...

Role of JNK isoforms in the development of neuropathic pain following sciatic nerve transection in the mouse

Conclusions:
JNK signaling plays an important role in regulating pain threshold: the inhibition of all of the JNK isoforms prevents the onset of neuropathic pain, while the deletion of a single splice JNK isoform mitigates established sensory abnormalities. JNK inactivation also has an effect on axonal sprouting following peripheral nerve injury. (Source: Molecular Pain)

2nd April was soon here & the date of my total abdominal hysterctomy! To say I was afraid was an understatement, not only of the operation itself but how I would feel afterwards. The hysterectomy is the end result of many years of prolonged bleeding and extreme pain every month, but not any more! Straight forward and soon home & 6 weeks on I m feeling 100% better. Despite a set back & a second stay in hospital due to an infection I am now well on the road to recovery & for anyone out there debating due to discomfort afterwards, not knowing what to expect, do it!
Conclusions:
Results suggest an influence of genetic variants of COMT gene in describing the variation in pain after treatment for low back pain. Replication in large samples with testing for other pain related genes is warranted. (Source: BMC Musculoskeletal Disorders)

How are Brain Tumors Classified?

Discussion
Primary brain tumors are a diverse group of tumors that together form the most common solid tumors in children. It is estimated that there are 2500-3500 children diagnosed each year in the U.S. with a brain tumor. Brain tumor causes remain unknown and the best treatment has not been determined. Overall 5-year survival is ~70% but the rates are diverse depending on the stage and tumor type. Multimodal treatment is often essential including neurosurgery, oncology, radiation oncology, neuroradiology, endocrinology, rehabilitation and psychology. Unfortunately many children may have long-term or late sequelae because of the treatment, particularly for younger children. Tumor recurrence is also not uncommon.
Some common ways that brain tumors present include: headache, nausea/emesis...

It has been reported that remarkable and sustained activation of astrocytes and/or microgliaoccurs in cancer induced pain (CIP), which is different from neuropathic and inflammatorypain. The present study was designed to investigate the role of spinal Toll-like receptor 4(TLR4) induced glial neuroinflammation in cancer induced pain using a modified rat modelof bone cancer. The rat model of CIP consisted of unilateral intra-tibial injection with Walker256 mammary gland carcinoma. Nine days after Walker 256 inoculation, a robust activationof both astrocytes and microglia in bilateral spinal dorsal horn was observed together withsignificant bilateral mechanical allodynia. This neuroinflammation was characterized byenhanced immunostaining of both glial fibrillary acidic protein (GFAP, astrocyt...<div id="medworm"><p><b><i>MedWorm Sponsor Message:</i></b> Have a look at <a href="http://www.theorthopedicsdaily.com">The Orthopedics Daily</a>, the new orthopedics portal powered by MedWorm, with all the latest orthopedics news and research updated daily.</p></div>

Conclusion&nbsp;&nbsp;Intraspinal masses showing metal-like density in X-ray or computed tomography but in MRI showing only lipoma or cystic lesions,
not metallic characteristics, the differential diagnosis should include iophendylate (Pantopaque) cyst. Oil-based contrast
medium is believed to have the potential to make a syrinx formation via arachnoiditis, which can lead to severe neurologic
deteriorations, so even if the patients do not represent with an acute neurologic deficit, surgical total removal of remnant
material without leaking should be considered.

Conclusion&nbsp;&nbsp; The high rate of late infections with low-grade germs and the frequency of screw loosening signs made us suspect a lack of
integration at the bone-screw interface. Surgeons should be suspicious if the patient presents a combination of new or increasing
pain and signs of screw loosening, and aggressive revision is recommended in these cases.

Spinal Cord Stimulation in the Treatment of Cancer-Related Pain: “Back to the Origins”

Abstract&nbsp;&nbsp;Spinal cord stimulation (SCS) has been used in the treatment of chronic pain for more than 40&nbsp;years. The most common indication
for SCS in the USA is failed back surgery syndrome (FBSS). Interestingly, the first two spinal cord stimulators ever implanted
were in patients suffering from bronchogenic carcinoma and pelvic cancer, respectively. While cancer accounts for millions
of deaths each year in the USA, pain is often the first sign of malignancy. An increasing number of people suffer from cancer-related
pain each year and many receive suboptimal relief. Given the demonstrated value of spinal cord stimulation in the treatment
of neuropathic pain, spinal cord stimulation should be considered “earlier” as an adjunct to the treatment of cancer-related
pain...

We report a case, with 5 year followup, who underwent ACDF from C5 to C7 for cervical spondylotic myelopathy. He showed neurological improvement after surgery but developed discharging sinus after 2 weeks, which healed with antibiotics. He improved on his preoperative symptoms well for the first 2 months. He started developing progressive neck pain and myelopathy after 3 months and investigations revealed spondylodiscitis at C3 and C4 with erosion, collapse, and kyphosis, without any evidence of implant failure or graft rejection at the operated level. He underwent reexploration and implant removal at the operated level (there was good fusion from C5 to C7) followed by debridement/decompression at C3, C4 along with iliac crest bone grafting and stabilization with plate and screws after max...

Lysophosphatidic acid (LPA) is a bioactive lipid that serves as an extracellular signaling molecule acting through cognate G protein-coupled receptors designated LPA1–6 that mediate a wide range of both normal and pathological effects. Previously, LPA1, a Gαi-coupled receptor (which also couples to other Gα proteins) to reduce cAMP, was shown to be essential for the initiation of neuropathic pain in the partial sciatic nerve ligation (PSNL) mouse model. Subsequent gene expression studies identified LPA5, a Gα12/13- and Gq-coupled receptor that increases cAMP, in a subset of dorsal root ganglion neurons and also within neurons of the spinal cord dorsal horn in a pattern complementing, yet distinct from LPA1, suggesting its possible involvement in neuropathic pain. We therefore generate...

In their Seminar on low back pain, Federico Balagué and colleagues (Feb 4, p 482) conclude that (occupational) mechanical factors are unlikely to be independently causative of low back pain. This far-reaching conclusion is based on reviews of published epidemiological studies and on the relation between evidence of tissue injury on imaging and low back pain.
Before we respond to the issues raised by J H van Dieën and colleagues, we would like to point out a couple of minor inaccuracies in their letter. Our Seminar was not about “low back pain” (all-cause) but about non-specific low back pain, as defined in the opening paragraph. This is not just a semantic issue. There is an important distinction between the two, and one that is highly relevant in this context. The biological plausibility of a mechanical role in (some) back pain—on the basis of experimental or laboratory studies including those by van Dieën and colleagues—mainly concerns specific types of low back injury such as acute prolapsed disc, fracture, etc. (Source: LANCET)

Conclusion&nbsp;&nbsp; This study describes the largest cohort of biomechanical data of the cervical spine in young adult recorded to date. The findings
demonstrate that no correlation was found between neck pain, CROM and maximal torque in the study cohort. On this basis, we
conclude that the CROM and maximal cervical torque should not be used as indicators to measure the progress of chronic neck
pain in physiotherapy training and sports medicine for the young adult.

Conclusions&nbsp;&nbsp;Significant changes in morphology of the lumbar spine and intervertebral discs were found. It was revealed that the effects
of osteoporosis are not limited to the bone but also present in the intervertebral discs. Mid heights of intervertebral discs
were higher in the osteoporotic and osteopenic groups when compared to normal subjects along with the lowered mid heights
of lumbar vertebrae. It was also observed that stronger vertebral bones were associated with more disc and vertebral degeneration.

DUNEDIN, Fla., May 17, 2012 (GLOBE NEWSWIRE) -- Wolstein Chiropractic and Wellness Center in Dunedin, FL is raising awareness about the importance of prompt treatment for car accident injuries. According to Dr. Karen Wolstein, many patients suffer chronic back pain and neck pain due to the buildup of scar tissue following an accident injury. Prompt treatment ensures that the underlying injury properly heals, which minimizes scar tissue. Dr. Wolstein uses chiropractic care to restore alignment to the body. She frequently combines adjustments with complementary treatments like massage therapy, which may help promote internal healing and naturally relieve pain.
Conclusions. This study has shown that a significant reduction in the pain intensity and improvement in body posture can be achieved by the usage of the active physical therapy methods (Back School) in nurses who are experiencing chronic lower back pain.Relevance to clinical practice. The Back School programme when compared with the passive physical therapies (such as massage, ultrasound treatment, etc.) shows significant improvement in reduction in pain and greatly improves the posture of healthcare workers. The adoption of the Back School programme for the treatment of the healthcare workers with chronic low back problems should be a treatment of choice and standard that should be adopted when designing occupational healthcare policies and procedures.
Conclusion. Pain diagram patterns may help to increase the likelihood of various rheumatic diagnoses including polyarticular pattern and inflammatory arthritis, and there was high inter-rater reliability. However, testing the value of pain diagrams in addition to a referral note is necessary to determine if they have added value. (Source: Rheumatology)

This study reports a 54-year-old man who was a carpenter by occupation. He suffered from left chest and back pain and left pleural effusion. Peripheral blood showed granulocytosis and high serum titers of granulocyte&ndash;colony stimulating factor (G-CSF) and CYFRA. He died 20 months later. At autopsy, a pleural tumor located around the left lung and thickening of the pericardium, diaphragm, and esophagus by tumor infiltration was seen. The tumor proliferated in papillary and solid alveolar patterns by neoplastic cells. They were positive for calretinin, D2-40, CK5/6, HBME-1, G-CSF, CK19, and E-cadherin. He was diagnosed with G-CSF-producing epithelioid malignant pleural mesothelioma. (Source: International Journal of Surgical Pathology)

Conclusion.  Tarlov cysts have been described in the literature as producing paresthesias and genital sensory disturbances. Hence, at least some cases of PGAD might be considered to be a Tarlov cyst‐induced paresthesia. Based on the relatively high occurrence of Tarlov cysts currently observed in women who suffer from PGAD symptoms, it would seem advisable to suspect Tarlov cysts as a possible organic etiological factor underlying PGAD.
Conclusions&nbsp;&nbsp; The present study showed disappointing outcome of instrumented fusion for the treatment of failed back surgery syndrome in
terms of perceived recovery, functional disability and pain. Conservative management is probably more beneficial and, therefore,
more selective and careful assessment should be done in order to prevent unnecessary surgery.

Thus, the school is a potential environment for developing musculoskeletal disorders and implementing back care programs. OBJECTIVES: To evaluate the effect of an educational program on schoolchildren's knowledge regarding back pain prevention. METHODS: 392 students from 4th to 8th grade from a Brazilian state school took part in the study; 114 students (30%) were evaluated at follow-up. The back care program included pre- and post-intervention assessments and a follow-up assessment, as well as theoretical and practical lessons. The time interval between the pre- and post-intervention assessments was 9 weeks, and between the post-intervention and follow-up assessments, it was 2 years.
We know that complementary health approaches are often used to manage symptoms of an underlying disease or condition, such as neck or back pain, or arthritic or musculoskeletal pain&#8212;usually along with conventional treatments. Back pain, in particular, is the most common condition for which adults turn to complementary health practices. And it continues to be an important area of focus of NCCAM’s research.
Spinal manipulation may be used by chiropractors, osteopathic physicians, naturopathic physicians, physical therapists, and some medical doctors with a goal of relieving low-back pain and improving physical functioning. These health professionals perform spinal manipulation by using their hands or a device to apply a controlled force to a joint of the spine. Most often they also recommend self-care practices. But, what does the science tell us? (Source: NCCAM Featured Content)

Back pain is one of the most common health complaints, affecting 8 out of 10&nbsp;people at some point during their lives. The lower back is the area most often affected. For many people, back pain goes away on its own after a few days or weeks. But for others, the pain becomes chronic and lasts for months or years. Low-back pain can be debilitating, and it is a challenging condition to diagnose, treat, and study. The total annual costs of low-back pain in the United States&#8212;including lost wages and reduced productivity&#8212;are more than $100&nbsp;billion.
AbstractIt is generally accepted that the endocannabinoid system plays important roles in spinal pain processing. Although it is documented that cannabinoid‐1 receptors are strongly expressed in the superficial spinal dorsal horn, the cellular distribution of enzymes that can synthesize endocannabinoid ligands is less well studied. Thus, using immunocytochemical methods at the light and electron microscopic levels, we investigated the distribution of diacylglycerol lipase‐alpha (DGL‐α) and N‐acylphosphatidylethanolamine‐specific phospholipase D (NAPE‐PLD), enzymes synthesizing the endocannabinoid ligands, 2‐arachidonoylglycerol (2‐AG) and anandamide, respectively. Positive labeling was revealed only occasionally in axon terminals, but dendrites displayed strong immunoreact...

It is now universally recognized that neonates can experience considerable pain. While spinal lamina I neurons projecting to the brain contribute to the generation of hyperalgesia, nothing is known about their electrophysiological properties during early life. Here we have used in vitro whole cell patch-clamp recordings in rat spinal cord slices to determine whether the intrinsic membrane properties of lamina I projection neurons, as well as their synaptic inputs, are developmentally regulated during the early postnatal period. Projection neurons were identified via retrograde transport of DiI injected into the parabrachial nucleus (PB) or periaqueductal gray (PAG) and characterized at postnatal days (P)2&ndash;5, P10&ndash;12, P19&ndash;23, and P30&ndash;32. Both spino-PB and spino-PAG ne...

Abstract&nbsp;&nbsp;Recently, the sacroiliac joint (SIJ) has gained increased attention as a source of persistent or new pain after lumbar/lumbosacral
fusion. The underlying pathophysiology of SIJ pain may be increased mechanical load, iliac crest bone grafting, or a misdiagnosis
of SIJ syndrome. Imaging studies show more frequent degeneration of the SIJ in patients with lumbar/lumbosacral fusion than
in patients without such fusion. Using injection tests, it has been shown that SIJ pain is the cause of persistent symptoms
in a considerable number of patients after fusion surgery. Recent articles reporting on surgical outcomes of SIJ fusion include
a high percentage of patients who had lumbar/lumbosacral fusion or surgery before, although well-controlled clinical studies
are necessar...

Abstract&nbsp;&nbsp;Natural (−)-(aS, 7S)-colchicine, an alkaloid and toxic natural product is a secondary plant metabolite, obtained from Colchicum autumnale also known as ‘meadow saffron’. Colchicine, among the Indian medicinal plants, is contained in the corms of Colchicum luteum and the seeds of Iphigenia, to the extent of about 0.25 and 0.9&nbsp;% respectively. These plants are not available in sufficient quantities to warrant any
commercial utilization. Natural (−)-colchicine has only one stereogenic centre: carbon-7. The biosynthesis of colchicine involves
amino acid precursors, phenylalanine and tyrosine. It is used to treat rheumatic complaints, especially gout, also prescribed
for its cathartic and antiemetic effects and also in initial treatment for pericarditis. It is...

Michele Rosenthal’s memoir, Before The World Intruded, is the story of her struggle with a life-threatening illness and the trauma it created.
Rosenthal shares her battle with Stevens-Johnson Syndrome (SJS), leading us from her diagnosis at age 13 through 24 subsequent years of psychological repercussions. Rosenthal recounts the story of how she fought for years to overcome her PTSD, and managed to slowly climb her way out of a serious case of depression and anxiety, experiencing a rebirth along the way. Ultimately this results in a remarkable tale of personal strength and post-traumatic growth.
The book is divided into four sections: ‘Shock;’ ‘Confusion;’ ‘Clarity;’ and ‘Healing,’ with each of these representing a different stage in Rosenthal’s journey. The first, ‘...

    Abstract
Nitric oxide is a pain signaling molecule and exerts its influence through two primary pathways: by stimulation of soluble guanylylcyclase and by direct S-nitrosylation (SNO) of target proteins. We assessed in the spinal cord the SNO-proteome with two methods, two-dimensional S-nitrosothiol difference gel electrophoresis (2D SNO-DIGE) and SNO-site identification (SNOSID) at baseline and 24h after sciatic nerve injury with/without pretreatment with the nitric oxide synthase inhibitor L-NG-nitroarginine methyl ester (L-NAME). After nerve injury, SNO-DIGE revealed 30 proteins with increased and 23 proteins with decreased S-nitrosylation. SNO-sites were identified for 17 proteins. After sham s...<div id="medworm"><p><b><i>MedWorm Sponsor Message:</i></b> Have a look at <a href="http://www.theorthopedicsdaily.com">The Orthopedics Daily</a>, the new orthopedics portal powered by MedWorm, with all the latest orthopedics news and research updated daily.</p></div>

The objective of this study was to seek scientific evidence regarding the types of work-related conditions or illnesses in nursing, as well as the means of coping and preventing occupational diseases and accidents. An integrative review was performed on the electronic health databases using the following descriptors: occupational disease, occupational prevention, and nursing worker. Twenty-seven articles were found. It was realized that nurses have back pain and musculoskeletal injuries, suffer accidents with sharp-edged materials and endure stress and tension at work, in addition to exposure to environmental pollution and dermatitis.La búsqueda incesante de la realización de diferentes actividades provoca en el hombre un aumento de las cargas laborales, acarreando la aparición de enfer...

CONCLUSION: There is limited data regarding biological behavior of solitary fibrous tumors with extra-pleural localization, because they are rare tumors. They are generally asymptomatic and slow growing tumors and it is difficult to distinguish them from other mesenchymal tumors. These issues as well as the prior nine cases are discussed.

The NHS is set to use Botox injections to treat chronic migraines, it has been widely reported today. The muscle-paralysing injections are popular as a cosmetic treatment, but due to its nerve-blocking effects, Botox does have a role in the treatment of certain medical conditions.
The move to use Botox to prevent migraines is based on new guidance published today by the National Institute for Health and Clinical Excellence (NICE), which looks likely to come into force in the near future. NICE recommends that Botox can be considered as an option for the prevention of headaches for people who have chronic migraine (headaches on at least 15 days of every month, at least eight days of which are migraine) that has not responded to at least three prior preventative drug treatments.

In April, 2011, an 85-year-old man was referred to our hospital with suspected septic spondylodiscitis; he had acute immobilising back pain, raised body temperature, and high C-reactive protein (CRP) concentration. His medical history included chronic back pain because of advanced spinal degeneration, chronic renal insufficiency, and gouty arthritis. On physical examination, his lumbar spine and right flank were very tender. The distal interphalangeal joint of the right index finger was painful, warm, and swollen. (Source: LANCET)

(RCC). RCC is commonly metastatic to the skeleton but almost always produces lytic metastases, with only three prior reports
of sclerotic metastases identified in the literature. Sclerotic metastasis causing low back pain was the initial disease presentation
in two of the three patients in this case series and the first manifestation of metastatic disease in one. The most common
metastatic sites of RCC, i.e., retroperitoneal lymph nodes, lung, and liver, were not identified in any of the cases, and
skeletal involvement with epidural extension was the only site of metastasis in two. Pathologic specimens from all three cases
revealed RCC of high...

Conclusions&nbsp;&nbsp; All patients with subacute inflammatory back or neck pain showing suggestive radiological findings should be studied to rule
out TVO. If there is no clear evidence of tuberculosis from another location or indication for surgery, a percutaneous vertebral
biopsy should be performed. When TVO is suspected, all spinal or paravertebral tissue samples should be sent simultaneously
to pathology and microbiology laboratories for appropriate processing.

Abstract
Background&nbsp;&nbsp;Open reduction internal fixation (ORIF) of distal radius fractures via a volar approach involves surgical release of the overlying
pronator quadratus (PQ) muscle. Complete repair of the PQ, defined as full and stable replacement of the periphery of the
PQ back to its original anatomic location, is not always possible upon conclusion of the operation. Postoperative consequences
of incomplete PQ repair with regards to range of motion (ROM), grip strength, and complications are not well documented. It
was hypothesized that the completeness of PQ repair would yield no significant difference in the postoperative ROM, grip strength,
and incidence of complications.

Background:
In Western Australia (WA), health policy recommends encouraging the use of active self-management strategies as part of the co-care of consumers with persistent low back pain (LBP). As many areas in WA are geographically isolated and health services are limited, implementing this policy into practice is critical if health outcomes for consumers living in geographically-isolated areas are to be improved.
Methods:
In this prospective cohort study, 51 consumers (mean (SD) age 62.3 (+/-15.1) years) participated in an evidence-based interdisciplinary pain education program (modified Self Training Educative Pain Sessions: mSTEPS) delivered at three geographically isolated WA sites. Self report measures included LBP beliefs and attitudes (Back Pain Beliefs Questionnaire (BBQ); Fear Av...

AbstractNon‐specific low back pain is a frequent complaint in primary care, but the differential diagnosis for low back pain can be complex. Despite advances in diagnostic imaging, a specific pathoanatomical source of low back pain can remain elusive in up to 85% of individuals. Best practice guidelines recommend that clinicians conduct a focused physical examination to help to identify patients with non‐specific low back pain and an evidence‐based course of clinical management. The use of sensitive and specific clinical methods to assess the lumbar spine, sacroiliac and hip joints is critical for effective physical examination. Psychosocial factors also play an important role in the evaluation of individuals with low back pain, but are not included in this narrative review of physic...

The May edition of 'Eyes on Evidence' the free monthly e-bulletin from NHS Evidence covering major new evidence as it emerges with an explanation about what it means for current practice, includes an expert commentary on new data from a non-blinded three arm RCT comparing the efficacy of spinal manipulation performed by chiropractors, home exercise with advice, and medication (paracetamol and/or NSAIDs) for acute and subacute neck pain in both the short and long term. The US-based trial was conducted from 2001-2007 and involved 274 patients. The primary outcome was participant-rated pain, measured at 2, 4, 8, 12, 26, and 52 weeks after randomisation.&#160; The results showed that both spinal manipulation therapy and home exercise with advice led to similar ...

SummaryNeridronate is a third generation bisphosphonate with established efficacy in metabolic bone disease. In this randomized, open‐label study, 118 adults with β‐thalassaemia and bone mineral density (BMD) Z scores ≤−2·0 were randomized 1:1–500 mg calcium with 400 international unis (iu) vitamin D daily or 500 mg calcium with 400 iu vitamin D daily plus neridronate 100 mg intravenously every 90 d. Significant increases in BMD at the lumbar spine and total hip were noted in the neridronate group at 6 and 12 months from baseline (P &lt; 0·001), and values were significantly higher than the control group at both time intervals. Neridronate also significantly decreased serum bone alkaline phosphatase and C‐telopeptide of collagen type 1 levels from as early as 3 mont...

Conclusions: There was no significant difference in the change in CSA between whiplash patients and healthy volunteers after a 10-year follow-up period. In both groups, the cross-sectional area slightly increased at follow-up. In addition, there was no association between the change in CSA and clinical symptoms such as neck and shoulder pain. These results suggest that whiplash injury is not associated with symptomatic atrophy of the posterior cervical muscles over the long term. (Source: Injury)

In fact, more than one. And an old kettle that could be useful for someone, surely? My friend – who, like me, is totally normal – has stopped herself buying a fifth sewing machine; a&nbsp;neighbour has racks of clothes she has not worn for 20 years. Vintage, right? We are just waste not, want not. Not like those people on programmes such as The Hoarder Next Door, for these people are unwell. They have a mental illness, OCD, anxiety or depression.Currently, there is a glut of TV programmes about these sad, compulsive people who have to tunnel through their own junk. Why would anyone live like th...

Conclusions  In patients for whom spinal anaesthesia was applied, high stress coping seems to be a quality that helps patients manage the burden of surgery. It is desirable for the individual, as well as for the healthcare system, to enhance recovery by using intervention programmes designed to improve or manage stress coping, particularly for individuals with low stress coping capacity. This recommendation merits further investigation. (Source: BJOG: An International Journal of Obstetrics and Gynaecology)

Itch, also known as pruritus, is a common, intractable symptom of several skin diseases, such as atopic dermatitis and xerosis. TLRs mediate innate immunity and regulate neuropathic pain, but their roles in pruritus are elusive. Here, we report that scratching behaviors induced by histamine-dependent and -independent pruritogens are markedly reduced in mice lacking the Tlr3 gene. TLR3 is expressed mainly by small-sized primary sensory neurons in dorsal root ganglions (DRGs) that coexpress the itch signaling pathway components transient receptor potential subtype V1 and gastrin-releasing peptide. Notably, we found that treatment with a TLR3 agonist induces inward currents and action potentials in DRG neurons and elicited scratching in WT mice but not Tlr3&#x02013;/&#x02013; mice.
Abstract&nbsp;&nbsp;A 75-year-old female was admitted to our hospital with sudden back pain and right leg ischemia. Computed tomography showed
acute type A aortic dissection with the occlusion of the right common iliac artery. The patient was treated with ascending
aorta replacement and femoro-femoral bypass. Three hours after the operation, the patient went into a sudden shock. Electrocardiogram
showed ventricular tachycardia and ventricular fibrillation. Percutaneous cardio-pulmonary support was administered and coronary
arteriogram (CAG) was proceeded for evaluation of the coronary arteries. Although CAG revealed normal coronary arteries, intravascular
ultrasound showed mobile intimal flap at left main coronary artery trunk, suggesting dissection of the coronary artery. Percutaneou...

CONCLUSION AND IMPLICATIONSSP in the LC exerted analgesic effects on neuropathic pain through NK1 receptor activation and resulted in facilitation of spinal noradrenergic transmission. Accordingly, manipulation of the SP/NK1 receptor signalling pathway in the LC may be a promising strategy for effective treatment of neuropathic pain.
The paper by Viana et al1 analyses the relationship between Harlequin syndrome2 and headache. One facet of this interesting syndrome is the demonstration of pupillary signs by pharmacological studies explicable by the sudden onset of parasympathetic and sympathetic deficit, implicating an autonomic neuropathy mediated by an autoimmune process or viral infection.3 Sympathetic innervation of human cerebral and extracranial circulation is well documented. Parasympathetic innervation of cranial arteries has been established in rat, cat and monkey, but information on humans is meagre. Stimulation of the pre- or post-ganglionic fibres of the sphenopalatine ganglion in animals increases cerebral blood flow, probably via orbital rami from the ganglion looping back to the internal carotid artery, a...<div id="medworm"><p><b><i>MedWorm Sponsor Message:</i></b> Have a look at <a href="http://www.theorthopedicsdaily.com">The Orthopedics Daily</a>, the new orthopedics portal powered by MedWorm, with all the latest orthopedics news and research updated daily.</p></div>

YORKVILLE, Ill., May 6, 2012 (GLOBE NEWSWIRE) -- Advanced Physical Medicine of Yorkville announced that the practice has had significant success treating patients using decompression therapy to treat back pain. The practice uses the DRX9000 machine to help restore a herniated disk to its proper alignment in the spine. According to clinic director and chiropractor Dr. Brian Berkey, this helps to relieve pressure on nearby nerves, which can relieve chronic pain. Dr. Berkey recommends non-surgical spinal decompression therapy as an alternative to back or neck surgery for the treatment of bulging intervertebral discs, spinal stenosis, sciatica, chronic low back or neck pain, herniated disks and degenerative disk disease.
DES MOINES, Wash., May 6, 2012 (GLOBE NEWSWIRE) -- Des Moines chiropractor, Dr. Bonnie Verhunce, has had success helping patients overcome neck pain from injuries, stress, and strains. Dr. Verhunce, who runs Vitality Chiropractic, stated that neck pain stems from a variety of sources and the type of treatment needed depends on the source. Whiplash is a common neck injury that causes a sore neck and can also lead to headache, migraine and dizziness symptoms. Dr. Verhunce said she often sees stress and poor ergonomic working habits causing painful neck strains. She has had success treating these injuries through a combination of specific spinal adjustments and therapeutic exercises. (Source: Medical News (via PRIMEZONE))

This article is essential for anyone with an interest in the topic. For medical personnel in Australia who may have to deal with these injuries, it is a must-read. But as the following video from National Geographic points out, Irukandji syndrome can occur almost anywhere:

Thanks to Chris Nickson of Life in the Fast Lane to drawing my attention to this article. Speaking of LITFL, Mike Cadogan has an excellent review of Irukandji syndrome on that site, with great illustrations.

One of the biggest choices an expectant mother faces is how to handle the pain of childbirth. More than 60 per&shy;cent of American women choose relief in the form of an epi&shy;dural, a combination of local anesthetic and narcotic administered into the epidural space surrounding the spinal cord. Although most doctors believe that the injections are safe, a new study suggests that they may increase the risk that a mother will develop a fever during labor, which could, in rare instances, pose risks to her baby.
Int J Sports MedDOI: 10.1055/s-0031-1301330The aim of the present study was to determine the association between joint range of motion (ROM) and patellofemoral pain syndrome (PFPS) in young female dancers. The study population included 1 359 female dancers, aged 8-20 years. All dancers were clinically examined for current PFPS, and their joint ROM was measured at the lumbar spine and the lower extremities. 321 of the 1 359 dancers (23.6%) experienced PFPS. Prevalence of the syndrome increased with the dancer's age (p&lt;0.001). Dancers with hypo ROM in hip external rotation, ankle plantar-flexion, ankle/foot pointe, hip abduction, hip extension, and limited hamstrings and lumbar spine were significantly less prone to developing PFPS compared to dancers with average ROM: 19.2% vs. 26.2% (p=...

Authors: Robertson EK, Stanley C
    Abstract
The patient was a 25-year-old woman who was referred to a physical therapist for a chief complaint of pain at the base of the occiput and left upper quarter region, as well as bilateral hand paresthesias. The findings of the history and physical examination were discussed with the patient's physician, who then ordered magnetic resonance imaging of the cervical spine, which revealed protrusion of the cerebellar tonsils 1 cm below the foramen magnum, consistent with a type 1 Chiari malformation.
DiscussionBy establishing the BOLD registry, we are creating a resource that contains patient-reported outcome measures as well as electronic medical record data for elderly patients with back pain. The richness of our data will allow better matching for comparative effectiveness studies than is currently possible with existing datasets. BOLD will enrich the existing knowledge base regarding back pain in the elderly to help clinicians and patients make informed, evidence-based decisions regarding their care.
Trunk lean gait modification and knee joint load in people with medial knee osteoarthritis: The effect of varying trunk lean angles

Conclusion:Increased trunk lean reduced medial knee load in a dose‐response manner. Slightly later achievement of peak trunk lean improved the load‐modifying effect of this gait strategy. No immediate symptomatic changes were identified. Future research should determine if longer‐term implementation of this gait strategy is feasible and whether it can modify disease symptoms and OA progression. © 2012 by the American College of Rheumatology (Source: Arthritis Care and Research)

I just wanted to say a big thank you for all the hints, tips, website and personal responses to my emails. I can&#8217;t tell you how well supported I have felt throughout what was a very harrowing lead up to my op. You may remember I had fought this op for 3 years and was terrified, so much so I had felt like doing something really stupid. I am 47 and had fibroids the size of cricket balls.
I had my op as planned on 3rd April at 5pm and was back out in Recovery at 7:15pm. My surgeon said the uterus was monstrous and he didn&#8217;t know how I had coped with it as long as I had, it was about a foot long, all knobbly and weighed 2kgs. He had carried out a sub total hysterectomy due to large bulky fibroids but had managed to preserve the lower part of my cervix and my ovaries at my r...

Abstract&nbsp;&nbsp; The interspinous distraction devices are used to treat variable pathologies ranging from facet syndrome, diskogenic low back
pain, degenerative spinal stenosis, diskopathy, spondylolisthesis, and instability. The insertion of a posterior element with
an interspinous device (ISD) is commonly judged responsive to a relative kyphosis of a lumbar segment with a moderate but
persistent increase of the spinal canal and of the foraminal width and area, and without influence on low-grade spondylolisthesis.
The consequence is the need of shared specific biomechanical concepts to give for each degenerative problem the right indication
through a critical analysis of all available experimental and clinical biomechanical data. We reviewed systematically the

In this study, we characterized the expression of Wnt signaling proteins in the spinal cord dorsal horn (SCDH) of EAE mice,
by immunoblotting and immunostaining. The EAE model was created by immunization of adult mice (C57BL/6, 10 weeks) with myelin
oligodendrocyte glycoprotein (MOG) 35–55. Robust mechanical hyperalgesia and allodynia were developed in both fore- and hindpaws
of the EAE mice. Wnt3a, a prototypical Wnt ligand for the canonical pathway, was significantly increased in the SCDH of the
EAE mice. Another key protein in the canonical pathway, ß-catenin, was also significantly up-regulated. In addition, Wnt5a,
a prototypic Wnt ligand for the non-canonical pathway, and its receptor (co-receptor) Ror2 were also up-regulated in the SCDH
of the EAE mice. We further found that...

Eric K. Robertson, Chris StanleyThe patient was a 25-year-old woman who was referred to a physical therapist for a chief complaint of pain at the base of the occiput and left upper quarter region, as well as bilateral hand paresthesias. The findings of the history and physical examination were discussed with the patient&#39;s physician, who then ordered magnetic resonance imaging of the cervical spine, which revealed protrusion of the cerebellar tonsils 1 cm below the foramen magnum, consistent with a type 1 Chiari malformation.
This report describes one case of sporadic renal hemangioblastoma happened in a16-year-old Chinese female patient, presenting with hematuria, and low back pain.Histologically, the tumors were circumscribed, and composed of sheets of large polygonalcells traversed by arborizing thin-walled blood vessels. The diagnosis of hemangioblastomawas confirmed by negative immunostaining for cytokeratin, and positive staining for alpha-inhibin, S100 and neuron-specific enolase (NSE). This benign neoplasm which can bemistaken for various malignancies such as renal cell carcinoma, epithelioidhemangiopericytoma and epithelioid angiomyolipoma, deserves wider recognition for itsoccurrence as a primary renal tumor.Virtual slidesThe virtual slide(s) for this article can be found here:http://www.diagnosticpat...

Among health care professionals, the critical role of radiology in patient care is obvious. For example, most emergency department patients receive some form of imaging when presenting with abdominal pain []. Among the general public, however, the radiologist's role is less clear. There is confusion as to the role of the radiologist, technologist, and even referring physician in image interpretation []. Furthermore, many people do not realize that radiologists are doctors []. The lack of general knowledge as to the importance of radiologists continues to have serious implications, particularly among policymakers []. One method of addressing this problem has been the ACR's Face of Radiology campaign, designed to educate the adult public that radiologists are doctors who are highly trained a...

    Abstract
Spinal dural arteriovenous fistulas (AVFs), the most common type of spinal cord vascular malformation, can be a challenge to diagnose and treat promptly. The disorder is rare, and the presenting clinical symptoms and signs are nonspecific and insidious at onset. Spinal dural AVFs preferentially affect middle-aged men, and patients most commonly present with gait abnormality or lower-extremity weakness and sensory disturbances. Symptoms gradually progress or decline in a stepwise manner and are commonly associated with pain and sphincter disturbances. Surgical or endovascular disconnection of the fistula has a high success rate with a low rate of morbidity. Motor symptoms are most likely to improve after treatment, followed by se...

This study appears to confirm that neurological status before treatment, anatomical location of the fistula, and symptoms at presentation are all factors that can predict outcome. Early diagnosis of spinal dural arteriovenous malformations in the lower thoracic area in patients with an Aminoff scale score &lt; 3 was associated with the most favorable outcome.

Short-term help for low back pain may come in the form of massage therapy. Seattle researchers led by Daniel Cherkin, Director of Group Health Research Institute, randomly assigned 401, mostly middle-aged white women, with chronic low back pain to receive relaxation massage, structural massage, or usual medical care, including pain medications, anti-inflammatory agents, muscle relaxants, or physical therapy. Massage therapy was delivered in hour-long treatments weekly for 10 weeks. After 10 weeks, over one-third of the massage patients said their back pain was much better or gone; this was reported by only 1 in 25 patients who received standard care. Overall, those who had either relaxation or structural massage took fewer anti-inflammatory medications, were more active, and spent fewer da...

Authors: Khan MA, Garcia-Kutzbach A, Espinoza LR
    Abstract
ABSTRACT:: Treatment of ankylosing spondylitis and related disorders has been revolutionized by the advent of biological therapy, especially tumor necrosis factor-α inhibitors. Nonsteroidal anti-inflammatory drugs, however, remain the first line of treatment. Evidence has accumulated that nonsteroidal anti-inflammatory drug therapy of ankylosing spondylitis and related disorders is effective in controlling several of the clinical manifestations seen in these disorders, particularly pain, physical function and perhaps progressive spinal fusion. What needs to be proven, however, is the long-term safety profile of these drugs.

DISCUSSION: More than half of the sample (n = 134) provided their own bowel management consisting of digital stimulation, suppositories, and other aids; 8% (n = 19) had a colostomy. Regardless of the bowel management program 54% (n = 127) were satisfied with current methods. Although time reported to complete bowel programs ranged from 1 to 120 minutes, there was no difference in rating of satisfaction with time. There was a statistically significant difference between those satisfied and dissatisfied with current bowel management and quality of life; those satisfied demonstrated a higher quality of life on three subscales, work function (p = .021), bowel problems (p &lt; .001), and social function (p &lt; .001). Those dissatisfied with their bowel program perceived a lower qu...

Abstract: Inhibitor kappa B kinase (IKK)-mediated nuclear factor-kappa B (NF-κB) activation is a major pathway for transcriptional control of various pro-inflammatory factors. We here assessed whether activation of this pathway specifically in primary nociceptive neurons of the dorsal root ganglia (DRG) contributes to the development of nociceptive hypersensitivity. Mice carrying a cre-loxP–mediated deletion of inhibitor kappa B kinase beta (IKKβ) in DRG neurons were protected from nerve injury–evoked allodynia and hyperalgesia. This effect was mimicked by systemic treatment with an IKKβ inhibitor but was not observed upon specific inhibition of IKKβ in the spinal cord, suggesting a specific role of IKKβ in the peripheral neurons. The deletion of IKKβ in DRG neurons did not affec...

Eighty-five-year-old Hannah is brought to your emergency department by EMS after she tripped over her cat and fell onto a wooden chair. She complains of pain in the left rib area, and “trouble breathing.” Although no paradoxical chest movement is noted over the left rib cage, it is noted that the left anterior and lateral chest has significant bruising and crepitus on palpation. Hannah has decreased breath sounds on the left, seems to be splinting when she breathes, and is using some accessory muscles. Further examination reveals no head, neck, or upper extremity pain. Her left upper abdomen is tender. She has shortening and rotation of the left leg, her pelvis is stable, and color, motion, and sensation (CMS) of the leg is intact. She has no injury to the right side or back. There was...

We describe a 7-year-old boy with an atypical presentation of reduction en masse after hernia reduction. The patient was a previously healthy child who presented with a mildly painful right inguinal mass of 1 day duration. An incarcerated hernia was noted, and after reduction, the pain dissipated for a short period, only to return with pain in the right lower quadrant and generalized peritoneal irritation. At surgical exploration, a reduction en masse was definitively diagnosed, and the incarcerated bowel was released from the hernia sac. (Source: Journal of Pediatric Surgery)
